Missed/No Show/Cancellation Appointment Policy:
Participating in the treatment process is an important part of meeting your service goals. I value the time that you have reserved for this process and ask that you make every effort to keep scheduled appointments. This will help assure my ability to meet the needs of all clients. Card on File:
Patients are required to keep a credit card on file to make the checkout process easier, faster, and more efficient. You will receive your Explanation of Benefits (EOB) from your insurance carrier once your claim has been processed, detailing the charges and payments made on your behalf.
At check-in you will submit the credit card of your choice, including your Flexible Spending Account (FSA) or Health Savings Account (HSA) for any copayment due. Your credit card information will always be fully protected by our off-site, card-processing system.
Good Faith Estimates:
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Leave of Absence and Disability Forms:
You must be a regular participant in services for at least 8 weeks prior to requesting forms/paperwork from or complete at least eight (8) therapy visits prior to requesting forms/paperwork from therapist to allow the time to understand your situation and to make an appropriate determination for completing the forms or paperwork. Completing documentation is at the discretion of the prescriber or therapist, and is not guaranteed. The below provides examples of forms or paperwork that we may complete. Note that the list should not be treated as a comprehensive list.
If you request any letters, forms, or any other paperwork to be completed, such as FMLA or disability forms, please be advised that there is a fee for paperwork. My fee is $150.00 per hour. FMLA paperwork generally requires a minimum of 30 minutes to complete, due to the need for supporting clinical documentation. Short-term disability often takes longer to complete, and may require additional assessments beyond my regular intake evaluation. The time required to make copies or prepare and send faxes, and any other administrative business (e.g. preparing releases of information or requests for records; phone calls to lawyers or other non-clinical calls) not directly related to the provision of clinical services, will also be assessed based on a rate of $150.00, with a minimum fee of $25.00.
I will not complete any FMLA, disability, other paperwork or letters of support unless I have met with you for at least 6-8 sessions. I will also not complete any FMLA or disability paperwork if I do not believe I can support it based on what you have presented at intake and during sessions.
